Discussion 1 Donna

A64-year-old female lives in rural areas in the country just outside asmall town in Ohio. She gets very little support from her family. Shehas two daughters, one of who she hasn’t spoken to in over 3 years, andthe other daughter is involved in her life., but not as much as shewould like to be. This female sees a psychiatrist every week to helpwith the emotional struggles that she has going on. She also has a dogthat she loves very much.

Social determinants of this individual are she is on afixed income, social security disability due to her medical conditions, ahigh school diploma, lived in the same rural area for 36 years, and anabusive alcoholic mother growing up. Her father passed away when she wasvery young. She is not very active due to her health conditions and isvery depressed because she can’t get around and do the things she usedto and that she hasn’t spoken to her daughter in a couple of years.

Social support in health promotion for her has a familyto provide a positive support system. Continue to see the therapist canlead to positive outcomes. If she could find coping strategies likerelaxation activities, talking to family and therapist to express heremotions, and going out and meeting new people, her age group. Hence,she is not just sitting in the house, making her more depressed.

The relationship between social support and social determinants isthat if you have greater social support, less isolation, and greater interpersonal trust, you live longer and healthier lives than sociallyisolated (Family and Social Support, 2020). If you lack adequate socialsupport, you are particularly vulnerable to stress, leading to cardiovascular disease and unhealthy behaviors (Family and Social Support, 2020).

Thetwo examples of improved patient safety through informatics I will usefor this week’s discussion are monitoring systems and pharmacy systems. Iwas recently floated to the ICU for a day where all the patients areconstantly monitored for heart rate, blood pressure, and 02 saturationwhich allows the nurse or doctor, at a glance, to see how the patientis doing. All these systems are also set to alarm if a vital sign is outof range, either too high or too low. In this way the, “trackingenables trending and recognition of potentially reassuring or concerningpatterns that require further intervention” (Information, n.d., Topic1, para. 9). The pharmacy systems add to patient safety by ensuring thefive rights are followed every step of the way when dispensing amedication, administering a medication, and when the doctor isprescribing a medication. “The dispensing portion of a pharmacy systemcan dispense the proper dose of medication, including labeling thattracks the patient’s name and other necessary demographic information”(Information, n.d., Topic 1, para. 14). The dispensing aspect alsoprovides safety to patients when the medication is being removed by anurse by ensuring one is pulling the right medication and the rightdose. This safety is continued to bedside where the patients barcodedbracelet is scanned and each medication is scanned, which will alert ifthe medication, dose, or time of administration are wrong. Anotherexample of pharmacy systems providing patients safety is throughe-scribing, “This system improves patient safety with reduction oferrors related to handwriting or omissions and prevents tampering withhandwritten prescriptions given to patients” (Information, n.d., Topic1, para. 16).

Thereare many different examples of patient safety that has been implementedover the years. Numerous places now have pictures of residents to helpensure that the right patient is being given the proper medication,along with making sure that you have the right patient. Some patientshave dementia and will say yes to anything that you ask them. Having aform of identification allows nurses to keep from making this error.

“EHRs ensure that patients receive the right medication the firsttime to prevent allergic reactions to antibiotics or other medicines.They even warn physicians before prescribing a specific drug if it showsdangerous interactions with a medication already in use by thepatient.” (“4 Ways Health Informatics Improves Patient Care | UIC Health Informatics”, 2020)

Paper-basedrecords make safely managing a patient’s prescriptions difficult asthese medical records do not get consolidated across physicians,specialists, laboratories, and hospitals. You also have lab values. Having an EHR for lab values ensuresthat any labs that are not within a therapeutic range are called intothe nurses so that they can let the doctors know so that the properorders are given for the patient to remain safe while under the nurse’scare.

These are just a few ways of patient safety and improvements,but electronic health records tend to make nurse’s jobs more manageableand helpful. This allows us to spend more time making sure that ourpatients get the attention that they deserve and are safe while we carefor them.

I see many aspects of this report being applied in my daily practice as a

nurse. I was, as are all of my colleagues, encouraged to be leaders,

collaborators, and caregivers. I was recently placed in a leadership

role over safety and compliance in my department which requires me to

earn at least a bachelor’s degree, so here I am. Also, nurses drive

care policy here at my hospital through committee. I sat on the policy

committee here for two years and most of it is safety driven.

For my practice as safety and compliance coordinator I amexpected to mediate and collaborate. Our department has a practicecommittee that involves nurses, radiology technologists, and providers.We discuss procedural roles, new procedures, and issues that havearisen since the previous meeting. This is a committee that nursingasked for so all parties can be sure that they are performing in thesafest way possible. This stemmed from an incident where a physicianwas going to begin a new procedure that no one was familiar with.Nursing said no, not until we see the evidence for treatment andalternatives as well as intra-procedural and post op care. As it turnedout care was different than other similar cases and we didn’t start theprocedures until everyone was truly ready.

TeamSTEPPS® is a tool used to promote collaboration and improvecommunication among our team members (Agency for Healthcare Research andQuality, n.d.). A proper TeamSTEPPS® approach was used when nursingheld up the new procedure as described above. Our physicians, nurses,and technologists are required to have TeamSTEPPS® training. Thisempowers people to step up and speak up without fear of repercussions.

Being a full partner transcends all levels of the nursing professionand requires leadership skills and competencies that must be appliedwithin the profession and in collaboration with other healthprofessionals (“The future”, 2011, pg. 222). Nurses play an essentialrole in providing optimal health care while implementing policy changes,nurses are the backbone of health care delivery. Although, nurses maynot take on responsibilities of a full leadership role, I thinkleadership becomes fundamental when advancing your nursing career.

Working on a critical care unit I find exercising leadershipcompetencies are very predominant in this specific collaborativeenvironment. This type of leadership involves working with a criticalcare team while having mutual respect to complete emergent task on thefloor. Strong leadership on the part of nurses, physicians, and otherswill be required to devise and implement the changes necessary toincrease quality, access, and value and deliver patient-centered care(“The future”, 2011, pg. 251). As nurses we already adopt mostleadership competencies, such as patient advocacy, knowledge of caredelivery, safety and quality of care. All things considered, there’salways room for improvement to succeed in future leadership roles.
